Required Application Materials
1. Form of Physical Examination Record for Foreigners and Blood Test Record
Applicants who want to stay in China for more than 6 months should provide two copies of the Form of Physical Examination Record for Foreigners and Blood Test Record, which are printed by the Department of Health Quarantine of China.
2. Final schooling certificate and school report
Applicants should offer two copies of their notarized final schooling certificate and school report in Chinese or English version. For a student at school now, a statement given by the school to prove the situation is required.
3. A letter of guarantee from the guardian
Applicants should offer the original letter of guarantee signed by his/her guardian.
